The conceptual and software developments generated in the context of Epistemonikos database allowed Epistemonikos team to engage in several international research and implementation projects. The combination of technical capacities in software development, methodological expertise and a vast network of volunteers was specially valuable for this kind of endeavours.

Epistemonikos is a collaborative, multilingual database of research evidence and knowledge translation products that are usually referred as "evidence", according to the meaning given in Evidence-Based Health Care. The primary aim of Epistemonikos is to provide rapid access to systematic reviews and other types of health evidence. 

Epistemonikos was developed and is maintained by systematically searching electronic databases and other sources for relevant systematic reviews and overviews of reviews, which are screened and processed by a network of more than 500 collaborators. 

It now includes more than 150,000 systematic reviews for decision-making in health, and it is probably the largest database of this kind. 

 A unique feature of Epistemonikos is that it links together systematic reviews, overviews of reviews and primary studies, thus providing a highly efficient method for searching. 

In addition, it includes translations of the titles and abstracts of included records to facilitate use in different languages.

L·OVE is a platform developed by Epistemonikos foundation  that provides all of the research evidence (systematic reviews and primary studies) relevant for health decision-making in a specific condition, organised in PICO-question format and continuously updated.

The easy-to-navigate platform is designed to be intuitively navigated by question. Just select the population of interest, and all of the questions associated will appear, with all the evidence and more.

You can open the information as a matrix of evidence, a table that allows you to compare different reviews and their included studies.

You can easily download the information you need, or export to traditional citation managers, Software for guideline development.

PDQ-Evidence is a database that facilitates rapid access to the best available evidence for decisions about health systems and population health. It includes systematic reviews, overviews of reviews (including evidence-based policy briefs) and primary studies included in systematic reviews; primary studies included in those; and structured summaries.

The connections between the documents make PDQ-Evidence very easy and quick to search.

For example, each systematic review is linked to all of the studies included in the review, to overviews and policy briefs that include the review, and to other systematic reviews that include any of the same studies.

Interactive Summary of Findings

Summary of Findings (SoF) are tables suggested by GRADE group that summarize evidence in a succinct, transparent, and informative way.

They provide an estimate of the size of the effect of an intervention on each of the most important outcomes of interest and an assessment of the certainty of the evidence for each of those estimates.

The objective of interactive SoF (iSoF) tables is to improve understanding and use of evidence of the effects of healthcare interventions by allowing Producers of iSoF tables to tailor a presentation to a target audience and Users to interact with the presentation by viewing more or fewer outcomes, more or less information about each outcome, information about the absolute effects as numbers, words or graphs, cursor over explanations and links to more detailed explanations of basic concepts (e.g. “95% CI”) and specific content (e.g. a specific outcome such as a pain scale).

The purpose of Evidence to Decision (EtD) frameworks is to help groups of people (panels) making healthcare recommendations or decisions move from evidence to decisions.

The interactive Evidence to Decision framework (iEtD) tool  facilitates use of EtD frameworks by guideline developers, health technology assessment agencies, policymakers and managers.

One reason for slow progress in achieving the health Millennium Development Goals in low- and middle-income countries is the difficulty policymakers and others have in accessing high quality information about potentially effective interventions to improve health systems.

 We are searching global databases for systematic reviews of ways to organise, finance and govern the delivery of effective healthcare interventions. 

To make this information more accessible to policymakers and other stakeholders we are preparing structured summaries of relevant reviews.

Project Transform aims to significantly improve the long-term value and sustainability of Cochrane by piloting, refining, and scaling up innovations in content production in collaboration with other Cochrane projects.

Project Transform is funded through a Cochrane Game Changer initiative (2015-2017) and a National Health and Medical Research Council of Australia Partnership Project grant (2016-2018 / APP1114605).

It has 5 components, including the evidence pipeline, where Epistemonikos participates.

Evidence Pipeline will address the difficulty in finding reports of studies for inclusion in a review in a timely and reliable way. With ongoing growth in the volume and diversity of research outputs this issue is becoming ever more pressing.

The project will build an ‘Evidence Pipeline’ in which study citations identified through automated and enhanced centralised search activities, including Project Transform's Cochrane Crowd platform, are ‘triaged’ to the most relevant review group or review using machine learning technologies. It will also assign suggested study type flags automatically: RCT only to begin with, and other types (e.g. diagnostic test) in later releases. The objectives are to improve the efficiency by which study reports are identified and to support the inclusion of all relevant studies in Cochrane information systems.

MAGICapp is developed and maintained by MAGIC, a non-profit organization backed by an international team of doctors and researchers. Their goal is to improve the authoring, adaptation, presentation, dynamic updating, and dissemination of medical guidelines using the GRADE methodology. 

MAGIC aims to facilitate shared decision making between patients and providers. This is accomplished by developing decision aids and using guidelines directly in Electronic Health Records and decision support systems, all with structured data. 

Epistemonikos is now collaboration with MAGICapp development, and also integrating Epistemonikos database and the iEtD into this tool.

This collaboration is part of a broader collaboration of different actors in the medical knowledge ecosystem which aim to improve the flow of information.

About Epistemonikos

The Database

Epistemonikos database was co-founded in 2009 by Gabriel Rada (Director of Evidence-based Healthcare program [PROSABE- Programa de Salud Basada en Evidencia], Pontificia Universidad Católica de Chile) and Daniel Pérez (owner of Etiqs, a software development company in Santiago, Chile). The funding for developing the concept and generating several pilot versions came from research grants, and in-kind contribution from the Pontificia Universidad Católica de Chile. The database was officially released in 2012. 

From the beginning it was planned as a comprehensive, friendly and multilingual database of health evidence connected around a structured (PICO) question. The sustainability was based on a collaborative model.

The Foundation

Soon after the database was released it became clear that a new structure was needed. Some of the reasons were: 

  • To recognize the diversity of people contributing to the project, coming from different institutions and countries. 

  • To give the project independence from the University, in order to pursue a global reach.

  • To explore alternative options to secure sustainability. 

In 2013, with agreement of Pontificia Universidad Católica de Chile, Epistemonikos foundation (Asociación sin fines de lucro ’Epistemonikos’) was established as a not-for-profit organisation under Chilean law. Its mission is "to bring evidence closer to those making health decisions, through technology and innovation".
